Have the English become confused about how they greet one another?

Social anthropologist Kate Fox says that while most other nations have an agreed form of greeting, the English have become confused since they stopped using 'How do you do?' as a way of greeting new people.

Mal Fletcher was asked on BBC Radio whether we've become confused on how to greet one another, and it is that language changes over time.
